How to refer your patient

I work in partnership with medical professionals to help your patients improve their lives and health. I rely on your documentation of diagnoses. Your most recent office note stating this diagnosis is usually sufficient.  I do need results of negative evaluations that have ruled out conditions such as MS, seizures, or cancer recurrence in my work with your patients who have functional or somatization disorders. I do not need imaging of any kind.

Communication

With your patient’s permission, I will send you my initial Diagnosis and Treatment Plan. Additional follow up will be as needed. A treatment summary may be provided upon discharge. I welcome your collaboration via telephone or email.
